Dr. Mendel Singer of the Case Western Reserve University School of Medicine is developing a program to improve the health of Orthodox Jewish families, and needs some feedback from the community to design an effective program. Please take a few minutes to help us learn about certain health needs in the Orthodox Jewish community – and maybe win $25! A brief online survey is being conducted by the Jewish Community Health Initiative at Case Western Reserve University (www.JewHealth.org), under its director, Mendel Singer, PhD MPH. Results will be used to develop programs to improve the health of families in the Orthodox Jewish community.
Upon completion of the survey, you have the option of providing your contact information so that you may be entered in a raffle for one of three online $25 gift cards for Amazon.com or for a local Jewish bookstore/gift shop of your choice. Your contact information is not saved with your survey responses, so we cannot link your personal information to how you answered the survey. That is why you will be taken to a 2nd survey to provide your contact information. It is only used for the raffle. If you choose not to participate in the raffle, you can simply exit the page after submitting your survey responses.
